There is one product in the menopause toolkit that every woman—regardless of her age, cancer history, or fear of hormones—should know about. Vaginal Estrogen.
We need to re-brand this. Calling it “HRT” scares people. It is not HRT. It is “Face Cream for the Vagina.” Just as you put moisturizer on your face to keep it from cracking, you need to put estrogen on your vaginal tissue to keep it from atrophying.
The Condition: Genitourinary Syndrome of Menopause (GSM)
Unlike hot flashes, which eventually stop, vaginal aging never stops. It gets progressively worse forever unless you treat it. Without estrogen, the vaginal tissue becomes thin, pale, and dry. The pH changes. The good bacteria die. This leads to GSM.
- The Vagina: Dryness, burning, itching, and painful sex (feeling like “sandpaper” or “shards of glass”).
- The Bladder: Urgency (feeling like you have to pee constantly), leaking, and Recurrent UTIs.
The UTI Connection (The Hidden Killer)

- It does not require progesterone protection (because it doesn’t thicken the uterine lining).
In fact, the American College of Obstetricians and Gynecologists (ACOG) states that vaginal estrogen is safe even for most Breast Cancer Survivors (including those on Tamoxifen), provided they coordinate with their oncologist.
The Options: Pick Your Vehicle
- The Cream (Estrace): Messy but effective. You use an applicator to insert it 2–3 times a week. Bonus: You can rub it on the external vulva/clitoris to help with external itching.
- The Tablet (Vagifem/Yuvafem): A tiny pill (smaller than a tic-tac) inserted with a disposable wand. It dissolves inside. Less mess, very convenient.
- The Ring (Estring): A flexible silicone ring you insert yourself. It stays up there for 90 days, slowly releasing estrogen. You don’t feel it (even during sex). This is the ultimate “set it and forget it.”
- The Suppository (Imvexxy): A gel-capsule that melts.
The Rule: Use It or Lose It
You have to keep using it. If you stop, the atrophy returns within weeks. This is a “forever” maintenance product, just like brushing your teeth. It preserves your plumbing, protects your bladder, and keeps intimacy possible well into your 80s.
